  1. I understand that submission of this application does not guarantee its acceptance by REGAP and that I must make two trips to the REGAP kennel in Mendota, IL; the first - to complete the interview process with all human and canine members of the household and to choose a greyhound, and the second - to actually pick up the greyhound.
  2. I understand that the fee for adopting a pet greyhound is $275.00, and that this fee includes spay/neuter, rabies/DA2PP vaccinations, tick panel, worming, heartworm test, dental cleaning, grooming, and a $10 AVID microchip activation fee.
  3. By clicking the "Submit" button, I certify that the above information is true and complete, and grant permission to REGAP of Illinois to contact and verify the references provided.
